I'm here for you when you need me. Call me today to schedule an appointment or complete our intake form online to create your patient portal by clicking here. Once I receive this intake form, I will email you an email inviting you to the portal to complete all of your new patient forms. Once I receive those new patient forms and a copy of your DL/INS card, then you will be offered the next available appointment which is typically within 1-2 weeks of all information being completed. You may also send a text message to the clinic phone number for another quick option to confidentially communicate with the clinic.
